Following a care plan from assessment to review

The care planning cycle is the organising theme of this course. You study how individual needs inform a plan, how goals and outcomes are set, and how the plan is implemented and reviewed. Involving the person and their family is included alongside the work of care staff.

The 40-minute curriculum is intended for workers who create, deliver or review plans in residential, domiciliary and community care. It offers a way to revisit the stages of planning and the contribution of each person involved.
